Physical Exam and Blue Card Information
Students must be seen and cleared by a physician in order to participate in Sanger High School Athletics.

How to become eligible:
- Print and have your physician complete this form.
- Email the completed form tomatilde_salazar@sangerusd.net or drop it off in the SHS Student Center.
- Once your physical is on file in the Athletics Office, you need to pick up a blue card for each sport you will participate in before the season and turn it into your coach.
***Your physical will be valid for 1 year.
